Porting Customers: I certify that I have the authority to switch this mobile service number to TeleChoice and that I have been advised that switching to TeleChoice may result in disconnection of the services (including value added services) relating to your mobile service number and finalisation of my account with my current mobile service provider. I acknowledge that the mobile number I am transferring to TeleChoice may be supplied to other carriers for the purpose of call and message routing, complaint handling and customer network fault management.

I acknowledge that I have been advised by TeleChoice that there may be costs and obligations associated with my existing service and the switching of this mobile service; and that there may be an existing contract with my current mobile service provider which may require me to pay them an early termination fee to cancel my current mobile service. TeleChoice may give information about you to a credit reporting agency to obtain a consumer credit report about you and/or allow the credit reporting agency to create or maintain a credit information file containing information about you. This information may include the details given by you on this form (including your driver’s licence number), the fact that you have applied for credit, information regarding any payments that are more than 60 days overdue or any cheques which have been dishonoured more than once or information that, in the opinion of TeleChoice, you have committed a serious credit infringement (that is, acted fraudulently or shown an intention not to comply with your credit obligations). This information may be given before, during or after the provision of credit to you. This disclaimer applies whether or not your application for credit is approved.
